Metabolic Complications of HIV Disease

The metabolic and morphologic abnormalities that complicate the management of HIV infection in the current therapeutic era include; dysregulation of lipid and glucose metabolism (hypertriglyceridemia, low HDL cholesterol, hypercholesterolemia and insulin resistance) peripheral fat wasting and central fat accumulation, lactic acidosis and lactic acidemia, and reduced bone mineral density and avascular necrosis of hip and shoulder. The inter-relationship between these clinical problems and the role of specific antiretroviral agents and HIV infection per se in their pathogenesis remains the focus of intense clinical investigation. This session will review recent data on the epidemiology, pathogenesis, potential long term sequlae and approaches to the clinical management of each of these problems. Case histories will be used to highlight difficult management dilemmas.
